Our medical clinic operates with a multi-disciplinary approach, involving a team of dedicated providers with varied and complementary skills and qualifications. This means that care is provided by family doctors, nurses, counsellors, or social workers. We provide care in the clinic, at home, and at long term and hospital based care. Our health care services are generally only available to registered and active clients of REACH. The health care program at REACH is funded by Vancouver Coastal Health to provide quality care to individuals and families with complex health and social issues.
Most visits are by appointment. If you are in need of care during clinic hours, you will be able to see a care provider in a timely manner. This can help avoid emergency room and walk-in clinic visits for conditions that can best be addressed by your primary care provider. We try to accommodate as many patients as possible every day and being on time helps us do this. In the case of lateness, your appointment will be reduced by the amount of time you have missed. If you are more than 15 minutes late you may be asked to wait or reschedule. If you need to cancel your appointment, please contact us at least 24 hours prior to your appointment.
We have a doctor "on-call" 24/7 to provide after hours-care for urgent issues. We also urge that you contact the Nurse Information Hotline (dial 811) for health related information and support outside of clinic hours. Unless you feel that your health concerns are life-threatening, there is no need to go to the emergency room until you have spoken to one of the nurses from the Nurses hotline or the doctor "on-call." This may save you the trip or a long wait in emergency.

While it is preferable that patients come into the clinic for medical care, under certain circumstances, house and long-term care visits are possible. Our physicians admit to St. Paul's and St. Joseph's and take an active role in hospital-based care.
REACH accepts many of our patients through referrals from other community agencies, but patients can self-refer as well. If you are a community agency looking to refer patients to REACH, or an individual in East Vancouver, please complete our patient application form and return to email@example.com.
Our MOAs keep the clinic running smoothly and communicate with you and our health care professionals. They will gladly make an appointment with a health care provider for you, assist you with referrals, and answer any questions you may have. Feel free to contact them by phone at 604-254-1354.
Our doctors take a comprehensive approach to providing high quality primary care. They are a group of dedicated and compassionate professionals who create a safe and respectful environment to address all your health care needs.
Registered Nurses are specially trained to assess, diagnose and treat minor and acute conditions and illnesses. They are also trained to provide preventative health care by doing Pap tests, immunizations, routine infant growth and development visits, birth control counselling, and sexually transmitted infection testing and follow up.
Our Licensed Practical Nurses (LPN) work to ensure the efficient operation of the medical clinic, they will measure and record your vital signs including weight, height, and temperature. All of our nurses work collaboratively with your physician to ensure complete care.
Our allied care professionals provide supportive counselling and social work services to REACH patients. Our in-house social worker is available to see you on an individual basis to provide short-term supportive counselling, disability supports, and provide information and referral to external community resources. Our registered clinical counsellor is now seeing patients to provide individual or couples therapy.

The REACH Basics for Health Program is a referral based program at the REACH Community Health Centre that works to support the social and economic needs of patients of the REACH Medical, Dental, and Multicultural Family Centre. The program operates with the understanding that the factors affecting a person’s health and wellbeing go beyond primary health care. Volunteers will work with patients to connect them with the appropriate resources in many areas – housing, income security, employment status, food security, child development and social services and more, in order to target underlying barriers to good health.
A dedicated team of trained and supervised community volunteers is available to support you with meetings your health goals.
